Summary: This perspective traces the history of DNDi from its origins in the Drugs for Neglected Diseases Working Group and the Fatal Imbalance report, to the thirteen treatments for six neglected tropical diseases achieved so far thanks to local expertise, regional and disease-specific research platforms, and committed pharmaceutical partners and funders. It also looks forward to the potential for using host-directed therapies and biologics, and the impact of the climate crisis on neglected tropical diseases. It finishes with an open invitation to potential partners in drug discovery, industry, clinical trials, and access to join DNDi in bringing the best science to the most neglected.
